Job summary

We are looking for an experienced, skilled, and motivated Advanced Practitioner to join our newly established Specialist Ventilatory Community Home Visits Service, part of the RSSC Home Mechanical Ventilation Service. This innovative, patient centred service ensures that individuals receive tailored, proactive, and flexible support at the right time, in the right setting, from the right team.

We are seeking an enthusiastic, dynamic, and forward thinking registered professional working at an advanced level across the four pillars of practice. You will help establish a five day a week community service for RSSC patients. A full driving license is essential.

Main duties of the job

As an Advanced Practitioner, you will play a key role in managing patients with complex home mechanical ventilation needs, exercising advanced clinical expertise and autonomous decision making across the four pillars of advanced practice:

Clinical Excellence

  • Conduct advanced clinical assessments for patients with complex ventilation, airway, and sleep related needs.
  • Develop and implement individualised management plans using expert clinical reasoning.
  • Provide specialist advice on ventilation and airway management.
  • Autonomously interpret complex clinical data and refer patients on when necessary.
  • If qualified, act as an independent prescriber in line with Trust protocols.

Leadership & Management

  • Lead and drive service development, improvement initiatives, and guideline creation within the Trust.
  • Manage and support clinical caseloads, ensuring appropriate prioritisation and delegation.
  • Attend and contribute to MDT meetings, ensuring timely communication.

Education

  • Provide expert advice, training, and clinical support to all RSSC staff and across the Trust.
  • Mentor and support colleagues in managing complex patient cases.

Research

  • Lead and support relevant audits, service improvement projects, and research in collaboration with other departments.
  • Maintain accurate treatment documentation in line with Trust and professional standards.
  • Work with the Lead ACP to evaluate service delivery.

This role will initially be based on the RSSC Day Cases to allow development of clinical practice.

Essential

  • Professional Registration with either NMC, GPhC or HCPC.
  • Advanced Clinical Skills qualification or working towards.
  • Non-Medical Prescribing if appropriate to registered profession / role.
  • Relevant role specific qualifications (i.e. Advanced Resuscitation -ALS/EPLS/ATLS or surgical skills qualification).
  • Willingness to work towards a master's degree

Desirable

  • Possession of master's degree in Advance Clinical Practice or

  • Proven experience of working with patients requiring invasive and non-invasive ventilation.
  • Proven experience in the management of patients with primary sleep disorders such as Obstructive Sleep Apnoea.
  • Proven experience of undertaking audits, incident investigation and change management skills.
