Job summary

This exciting post provides an opportunity to work within a multidisciplinary team of qualified and unqualified staff. You will assist the Safeguarding and Social Work team in collating and compiling safeguarding information and facilitate safe and timely discharge of patients from the hospital. You will have excellent proficiency in Microsoft programmes such as Word, excel, and PowerPoint to support the Safeguarding and Social Work team when preparing reports and documentation.

This role is part-time (30hours per week)

Main duties of the job

The Safeguarding and Social Care Coordinator:

*Supports the safeguarding team to collate, organise, and maintain safeguarding-related information and data, ensuring accuracy and confidentiality when preparing reports and other documentation.

*Assist the team by undertaking tasks to facilitate the safe and timely management of information and referrals internally and requesting packages of care from external providers accordingly.

*Will be required to keep detailed records of the processes and timescales.

*Will be expected to be able to interact with a patients, relatives, and colleagues in a professional manner, communicating confidential information in written, verbal and electronic formats where necessary.
