Our Social Work and Discharge Planning Team is a small, skilled, and friendly team working across the whole trust. We are looking for a qualified and experienced Social Worker as Social Work and Discharge Team Leader to lead and develop the service to meet patient and hospital's needs.

The Social Work service model at Royal Papworth Hospital is unique to the specialist nature of the hospital. The team provides; Specialist social work support- Working to address health inequalities for patients with chronic conditions arising from their social circumstances, arranging for safe and timely discharge from hospital and responding to safeguarding concerns in the hospital.

This role is full time (37.5 hours per week) but flexible working will be considered and the opportunity for an informal visit can be arranged. We are solely hospital based with working from home opportunities once a week. The ability to drive is therefore not essential for this role.

Main duties of the job

You will provide specialist social work and discharge services to Royal Papworth Hospital NHS trust as part of the Social Work and Discharge Team. The Social work and Discharge Planning Team Leader will have:

*Line management responsibilities for members of the social work & discharge team.

*Responsibility for co-ordination, provision and development of the social work functions alongside a Multidisciplinary Teams.

*Be able to assess, intervene and advise on safeguarding concerns.

*Responsibility for ensuring that patients can access the services and resources they need to successfully transition back to their homes and communities.

*The ability to work closely with patients, their families, and other healthcare providers to assess their needs and develop a plan for their ongoing care.

*Opportunity to attend external study days and conferences.

*Opportunity to participate and lead service development projects and audits.

*Involvement in the education and training of clinicians, nurses, and other health professionals.

  • Professional Registration -- Social Worker/ Social Work England registration
  • Evidence of professional study

Desirable

  • Advanced social work qualification e.g. Practice Teaching qualification, Masters.

Experience

Essential

  • Demonstrate experience in managing complex caseloads, autonomous practice and providing expert advice in a health or social care setting.
  • Experience of Safeguarding procedures and processes.
  • Awareness of anti-discriminatory and anti-oppressive practice.
  • Experience of working as a social worker post qualification.
  • Proficient in the use of Microsoft Office such as Word, Excel, Outlook, PowerPoint.
