Job summary
The LYPFT Learning Disability service provides multi professional, specialist health services for people over the age of 18 with a learning disability across community settings, respite and the intensive support team. The service works in partnership with key stakeholders including service users and carers, adult social care, third sector and private providers to deliver health care for people with highly complex health needs that may include behaviours that challenge services, mental health and/or dual diagnosis and complex multiple impairment / physical health needs.
An exciting opportunity has arisen for a nurse to join the East North East Community Learning Disability Team on permanent basis. We are looking for dynamic community nurses keen to work with learning disabled adults who require specialist health support due to vulnerabilities, complex physical health, sensory, communication and behavioural impairments.
The successful candidates will be based within the East North East CLDT. The service runs Monday to Friday 9am to 5pm.
The band 6 community nurse will provide direct clinical care to adults with a learning disability with the aim to improve service users lives and prevent admissions to specialist mental health and general health hospitals by ensuring that the right plans are in place.
Main duties of the job
The post holder will provide clinical and operational supervision to junior colleagues within the team, developing intervention toolkits for nurses as well as holding a caseload including care co-ordination to ensure care teams or family carers are supported to implement care and treatment as recommended.
The Learning Disability service has clear clinical governance pathways and supports professional and service development. As a band 6 there is an expectation to contribute or lead on service development, service projects and task groups. Robust supervision structures are in place with 1:1s with support from the Specialist Nurse Practitioner, peer support through weekly MDT meetings and community nurse meetings.
The candidate should have a passion for improving the health and well-being of adults with a learning disability, using their experience of leading and delivering high quality care from relevant experience. For this role you will need to be a compassionate and person-centred clinician who can carry out comprehensive, person-centred holistic assessments to a high standard. You will need to be committed to coordinating across multi- disciplinary and multi- agency teams to promote and advocate the health needs of adults with learning disabilities. We would value a candidate who has experience of working in a diverse population and promoting social inclusion.
