Job summary

An exciting opportunity has arisen for a community support worker, with the LYPFT Rough Sleepers Mental Health Service.

We are a new team within LYPFT and you will be part of shaping and developing the new service.

This newly developed team will have the time and space to deliver care directly to those service users most at risk of rough sleeping, using a trauma-informed approach.

Working collaboratively with the city's other street support teams, we will provide exceptional holistic treatment to those who need it most. Rough sleeping and homeless services in Leeds are unique and comprehensive and this team has a real opportunity to make a huge difference to those that needs support the most.

We are looking for a Community Support Worker who has experience of working with people with complex trauma, who may also be experiencing mental and physical health difficulties.

The successful candidate will work as part of a small Rough Sleepers Mental Health team and alongside partner services in the community, supporting people who are rough sleeping, or at risk of rough sleeping across the city of Leeds.

There will be training, supervision and support available from the Trust and within the team.

Main duties of the job

The successful candidate will work in the Rough Sleepers Mental Health Service and alongside partner organisations based in the community, supporting people who are rough sleeping or at risk of rough sleeping.

The Community Support Worker in the Rough Sleepers Mental Health Service will work in close collaboration with Mental Health Practitioners with tasks to include:

  • Provide direct support to service users sleeping rough in the community in the form of outreach support
  • Implement agreed specialist psychosocial interventions plans, which support the service user's care plan.
  • Demonstrate and role-model constructive person-centred approaches to the service user's family or care team to increase their ability to provide effective interventions.
  • Contribute to the assessment, planning and evaluation of care provided.
  • Work with partners across the city to work in an integrated way to provide a smoother service user experience
  • Access supervision within the team
  • Complete case notes and documentation effectively and within a timely manne
  • The working days are Monday to Friday. Flexibility is required to meet service and service user need. However, there may be a requirement for some evening hours in order to meet service demands.
