Job summary

An exciting opportunity has arisen to join an established but evolving mental health Care Homes Service. The LYPFT citywide Care Homes Team is seeking a Band 5 Occupational Therapist or Registered Nurse (MH) to join a small, friendly service that directly supports older people with complex dementia and/or mental health needs residing in a Leeds care home.

You will have post registration experience and be able to demonstrate an in depth understanding of the needs of Service Users with dementia and complex mental health needs. We shall provide you with specialist training and development to ensure you are fully supported in this role.

Broadly, the model of service delivery includes: care coordination; timely assessment, role modelling appropriate interventions, behaviours & techniques, education for care staff tailored to the needs of the individual service user, fast access to mental health, psychology and pharmacological advice.

Main duties of the job

The job requires successful applicants to undertake the role of Care Coordinator and to provide tailored, clinical assessment and intervention to care home residents & to support care homes with meeting the needs of more complex residents.

You will have the desire to enhance the wellbeing of others & promote recovery. You will work with your team to provide high quality person-centred care to our service users and provide written

recommendations and guidance for care home staff on approaches to care, including occupation and meaningful activity, aspects of personal care, therapeutic observations/interventions & maintaining safety/minimising risk.

You will manage a varied and complex caseload which will include completing holistic and specialised assessments and therapeutic interventions, and providing detailed risk management plans. You will liaise with colleagues and attend weekly multidisciplinary meetings.

Excellent caseload management skills are needed as you will be required to provide care co-ordination within the Community Mental Health Framework /CPA process. Service User and carer involvement is at the centre of our approach, and this should be reflected in your values and practice.

We welcome applications from people who have a strong commitment to working with older people using evidenced based practice and are prepared to develop their professional skills and knowledge.

The post holder must be able to act as a role model for others, and demonstrate a high level of clinical judgment, decision-making and communication skills.

You will be part of an MDT consisting of occupational therapists, nurses and psychologist, with medical support from Sector Consultant Psychiatrists and their colleagues. The service is commissioned to operate over 7 days, though the current core working hours are Monday to Friday. There may be a requirement to undertake some weekend work.

The right candidate will be supported into the team with an induction and then in developing and furthering their professional skills through reflection in appraisal objectives, management and clinical supervision and further training and development opportunities.
