In collaboration with Dementia UK, Leeds and York Partnership NHS Trust has a unique opportunity for a Registered Nurses (RN, RMNH, RNLD) to join the Admiral Nurse Accelerator Learning Programme (ALP).
The ALP, developed by Dementia UK, is designed to support nurses to become a dementia specialist Admiral Nurse and launch a new Admiral Nursing service across Leeds Teaching Hospitals NHS Trust (St James's University Hospital and Leeds General Infirmary).
Dementia UK is the specialist dementia nurse charity. Its nurses, called Admiral Nurses, provide life-changing support to families facing dementia.
As dementia specialists, Admiral Nurses help families manage complex needs, considering the person living with dementia and the people around them. They provide tailored clinical advice and psychological support; advise people about benefits and financial issues; support families in their caring role; and help build links with other health and care professionals.They also support other professionals through case management and the sharing of best practice.
Main duties of the job
The successful applicantwill be an NMC Registered Nurse,withpost-registration clinical experience of supporting families affected by dementia. They will demonstratecommitment to the ALP and a passion for dementia nursing. While this post is based within the Leeds Teaching Hospitals, the postholder will also work closely with the Liaison Psychiatry Service and receive management support from their operational lead.
Following the successful completion of the Accelerator Learning Programme the Transitions of Care Admiral Nurse will coordinate NHS discharge pathways and use their specialist knowledge to improve the quality, and experience of transitional care from the acute to care home or home setting.
The post holder will support people with dementia (PWD), their families, and professionals to ensure the right place destination on discharge, preventing placement breakdown and proactively pre-empt crisis.
They will case manage a cohort of PWD and their carers living with complex needs through transitions of care - inclusive of inpatient care, through the discharge process and beyond - with a designated period of post discharge support.
They will work with all relevant stakeholders as part of the wider multi-disciplinary team (including the care home sector) and bring a dementia specialism to the Integrated Discharge Team.

About the Accelerator Learning Programme
The ALP is a six-month blended learningprogramme. You will divide your time between study days (2.5 days a week) and developing a new Admiral Nursing service within Leeds teaching Hospitals (2.5 days)
You will:
- develop the specialist skills, clinical expertise and leadership skills needed to practise as a Band 7 dementia specialist Admiral Nurse
- receive dedicated support from the ALP Team, experienced Consultant Admiral Nurses and host line manager, who will support you to develop and launch a successful Admiral Nursing service
- have access to the Dementia UK Admiral Nurse Academy supporting specialist professional and practice development
- become part of a community of passionate specialist dementia nurse
You will receive ongoing support from Dementia UK during and after the programme to maintain and strengthen your clinical expertise. You will have the opportunity to access relevant postgraduate courses and to continue to develop professionally through the Admiral Nurse Competency Framework.
