Job summary

Leeds and York Partnership NHS Trust has a national and international reputation in Addiction and CoOccurring Mental Health Alcohol and Drugs (COMHAD) treatment. The COMHAD team, as part of LYPFT, works alongside other partners that make up Forward Leeds, a CQC outstanding rated organisation.

An exciting opportunity has arisen for a Band 6 Specialist COMHAD Practitioner to join the multidisciplinary team. The team consists of a Consultant Psychiatrist, Consultant Psychologist, Mental Health Nurses, Cognitive Behavioural Therapist, Social Worker, Specialist Health Visitor and a Specialist Midwife.

There are excellent specialist training opportunities such as Dialectical Behaviour Therapy and a focus on career development. The successful candidate will also have opportunities to facilitate specialist psychosocial group work and an involvement with service development.

Main duties of the job

The job role will include providing specialist addiction treatment, including psychosocial and pharmacological interventions, harm reduction, & risk management to service users with co-existing mental health and/or pregnancy needs. The LYPFT team is trauma-informed and offers a formulation driven, psychological model.

The successful candidate will hold a caseload as well as delivering psychosocial interventions to service users to support the wider service. Excellent training, supervision and support from an experienced team based in a clinical centre of excellence will be offered.

Planning and Organisational Skills Provide specialist clinical expertise with the provision of assessments, care planning evaluation, risk assessment and follow up to patients. Deliver a range of evidence led psychosocial interventions and support pharmacotherapies as appropriate with support from senior staff. Perform physical health checks and interventions such as venepuncture, immunisation, wound care, urine sampling. Participate in clinical supervision based on video recording of practice. Participate in continuous professional development. Undertake any other duties commensurate with the grade. Collaborate with primary care colleagues in improving care provided for this service user group.

Physical Skills(covers minimal demand for physical skills, standard driving/keyboard or advanced/highly developed physical skills such as hand-eye co-ordination, sensory skills, moving patients and surgical activities) Post holder will be able to demonstrate good key board skills Car driver is essential 6. Patient/Client Care Collaboratively plan and implement packages of care with service users in liaison with other multidisciplinary team members. Team meetings, regular clinical supervision, as well as ad hoc supervision will provide the opportunity to discuss service users and their specific needs. Provide advice and consultation on all aspects of addiction interventions relating to alcohol dependence and other substances to colleagues within the specialist team, Forward Leeds, LYPFT and Primary Care GPs, this will include psychological and pharmacological interventions

7. Policy and Service development Implementation(covers responsibility for development and implementation of policies and/or services i.e. implements in own work area, policy/service development across own area/directorate/organisation)

Responsible for Financial and Physical Resources(covers financial resources i.e. cash, cheques, budgets and physical resources such as clinical, office and other equipment, tools, instruments, personal possessions of patients/clients) To ensure that those responsible for financial and physical resources are made aware when equipment needs ordering.9. Responsible for Human Resources To ensure that all activity undertaken within the Specialist Addiction service adheres to Trust and Forward Leeds policies and procedures.

Responsible for Information Resources To complete System 1 records for service users in treatment with Forward Leeds To comply with Forward Leeds policy in relation to information sharing and recording with Humankinds SystmOne Management Information System. To maintain own paper and computerised records for line management supervision.
