Job summary

Would you want to be at the forefront of a new service aiming to provide "wrap around" care in a newly commissioned bed base? This is an exciting dynamic service that has been designed to meet the needs of older people with Dementia and we are looking to recruit individuals who have the passion, skills and experience to work with older people who have a Dementia.

We are keen to recruit into the following position

  • Registered Mental Health Nurse at Band 6

The is a 10 bedded unit for people who following discharge from hospital would present with behavioural and psychological symptoms of dementia and require 24 hour care, you will work in partnership with Leeds City Council and Leeds Community Healthcare.

The role of successful candidates will be to support staff who works permanently within the unit, providing support, advice and a range of person centred interventions based to meet the individualised needs of the person with dementia.

The team will operate alongside our trust care homes service under the same operating model with defined tasks, roles and responsibilities. We strongly recommend contacting the service prior to making an application.

Main duties of the job

The post would require you to support our partner organisations within a mental health specialist team using skills from your professional background. Mental health assessment, care planning and interventions will be core to your care coordinator role as well as partnership working and effective communication with family/carers. You will be required to arrange and contribute to multidisciplinary meetings and care programme approach meetings. You may role model or advise approaches to our partner organisations that help service users maintain good wellbeing and encourage meaningful activity as part of your role.
