Job summary

The Rehabilitation and Recovery service is an exciting and innovative service that uses a stepped approach to our inpatient care and has the additional resource of a transitional in-reach and outreach team (The Recovery Centre).

The service is focussed heavily on multidisciplinary and multi-agency team working with the service being made up of staff members from LYPFT, Community Links, Leeds Mind and Touchstone - all working in partnership under a managed network, within LYPFT. The service aims:

We are looking for a Band 3 Recovery Worker who is motivated and passionate about working in a rehabilitation and recovery model of care and will work as part of our Recovery Centre team.

The Recovery Centre is a multi disciplinary and multi agency team with staff members from LYPFT, Leeds Mind, Touchstone, Community Links and St Annes. The team provides in reach to service users within the inpatient rehabilitation units (Asket House and Asket Croft) and supports their transition into the community. The Rehabilitation and Recovery Service is established on strong principles of recovery, social inclusion and diversity.

The service provision is from Monday to Fridays between the hours of 8am and 8pm with flexibility depending on and responding to the needs of the service users.

Main duties of the job

The post holder will work alongside the Recovery Workers, Peer Support Workers, Care Coordinators and the wider team to support service users that present with complex needs to deliver specific components of prescribed care packages. Supporting service users in order for them to take control of their own lives and engage actively in their unique recovery process. The post requires effective liaison with service users, carers, and other professionals and agencies as required. It will involve the provision of direct physical and psychological based care, offering therapeutic time to a defined client group working at all times within the principles of recovery.

  • Work independently with users and carers in their homes towards a collaborative package of care provided through the CPA process and under the direction of the care coordinator or senior team members.
  • Participate in the delivery of Care plan including attendance and contribution within the meeting and the subsequent care plan.
  • Help individuals identify their own achievable and meaningful recovery goals and collaboratively set recovery objectives in line with service care pathway and supervision of senior team members.
  • Deliver therapeutic interventions to service users in a group setting
  • Promote the physical health and wellbeing for clients by facilitating and supporting access to relevant screening and information around medication, diet, nutrition, exercise, sensible drinking and smoking cessation
  • Involvement in the process of effective risk management including assessment, planning, intervention and ongoing monitoring of risk factors

Maintain accurate records of client care, informing senior clinicians of any changes in the clients health or social care. To contribute to the evaluation of care plans by reporting observations and interventions accurately and regularly in case notes. Also to make clear written records in relation to any task or clinical practice that relates to individual scare e.g. liaison with family members
