Job summary

An exciting opportunity has arisen to become the Patient Flow and Capacity Manager for the Older People's Service (OPS). You will manage a small team of people to oversee the flow of older people through the OPS, with focus on inpatient capacity and flow at The Mount hospital. The Mount has 4 wards, 2 for patients with dementia and 2 for patients with a mental health need. You will work closely with teams from within the organisation, partner providers such as Adult Social Care and Out of Area providers. You will work closely with the Urgent Care Teams to prevent admission and support timely discharge.

There will be an opportunity to take a lead role in the future development of bed management functions for older people as the Trust reviews and improves the current procedures.

Main duties of the job

  • To lead the facilitation of the optimum usage of beds to ensure that the Trust maintains capacity for admission and safe transfer/discharge of service users
  • To take a lead role in the creation of capacity to meet demand and liaise with clinical teams and matrons when insufficient demand is created
  • To lead on the review of delayed discharges across all OPS mental health wards, liaising with accommodation partners, Adult Social Care, Continuing Health Care and any other partner organisations to support transfer/discharge
  • To take a lead role in the future development of Patient Flow and Capacity Management within the OPS and the ongoing review of the effectiveness of the team, making recommendations for changes and improvements to the OPS Senior Leadership Team.
