So what is Community Transformation?
The Community Mental Health Transformation Programme a partnership of NHS organisations, Leeds City Council, the third sector and service users/people with lived experience coming together to transform how primary and community mental health services are currently organised and delivered for adults and older people with ongoing and complex mental health needs.
Our ethos is ensuring that people access the right care and support at the earliest point of need, providing the care and support closer to home so they can live as healthy and fulfilling lives as possible in their community.

We are developing and testing what we are calling 'Integrated Community Mental Health Hubs' where we continue to focus on delivering the highest quality care to complex individuals. We use a variety of interventions across sectors to assist in providing support and care whilst reducing risk and enabling our service users to manage their own risk behaviours. Main duties of the job
Work with colleagues to deliver and support the effective organisational change of staff across the system, offering support with inductions to support retention, Workforce and ESR and Budget work etc.
Work alongside the Executive and/or Senior Leadership Team to provide strategic input and challenge to future resourcing options and opportunities
Work with Community services, The Head of Strategic Resourcing and HR Business Partners to establish future skill requirements and gaps
Build constructive relationships with partners and providers to strengthen the future pipeline of talent
