So what is Community Transformation?

The Community Mental Health Transformation Programme a partnership of NHS organisations, Leeds City Council, the third sector and service users/people with lived experience coming together to transform how primary and community mental health services are currently organised and delivered for adults and older people with ongoing and complex mental health needs.

Our ethos is ensuring that people access the right care and support at the earliest point of need, providing the care and support closer to home so they can live as healthy and fulfilling lives as possible in their community.

We use a variety of interventions across sectors to assist in providing support and care whilst reducing risk and enabling our service users to manage their own risk behaviours. Main duties of the job

Work with colleagues to deliver and support the effective organisational change of staff across the system, offering support with inductions to support retention, Workforce and ESR and Budget work etc.

Work alongside the Executive and/or Senior Leadership Team to provide strategic input and challenge to future resourcing options and opportunities

Work with Community services, The Head of Strategic Resourcing and HR Business Partners to establish future skill requirements and gaps

Build constructive relationships with partners and providers to strengthen the future pipeline of talent

Job responsibilities

As Strategic HR Community Transformation Lead you will create some structure and resilience around workforce planning, HR, recruitment and retention for the new Community Transformation project, identifying future skill requirements and leading the creation and development of new roles, career pathways and establishing a future pipeline of talent. You will lead the wrap around and pastoral support for the workforce, using relationships and data intelligence to inform retention strategies and programmes.

You will also support the development and implementation of the pilot sites, ensuring change for the future and a new way of working

Employee Relations

  • Work in close partnership with Leeds Community Heatlhcare NHS Trust, 3rd party sectors including the local authority to coordinate and manage the project
  • Develop positive relationships with the HR team(s), working closely with the Recruitment and Resourcing Manager, and the Recruitment Team Manager to support operational delivery
  • Develop positive cohesive working relationships with staffside, ensuring early engagement with new career pathway development.
  • Manage any organisational change in line with the transformation programme

System Leadership

  • Represent the Trust at system and ICS level in relation to workforce planning for the Community Transformation unit and future resourcing strategies, working collaboratively to improve the pipeline of talent into Health and Social Care.
