Job summary

This is an exciting opportunity within our working age acute services for a Consultant to work alongside three other consultant psychiatrists in the Crisis Resolution Intensive Support Service (CRISS) in Leeds.

The post holder will be based within one of the 3 home based treatment hubs of the wider CRISS in the South Intensive Support Service at Aire Court Community Unit in Middleton.

The CRISS model became operational in March 2019 and now provides assessment and intensive support for acute mental health problems in working age adults, as well as for older adults out of hours, with the support of the OPS Consultant on call.

The model's service specifications were developed following an extensive community redesign task project which included stakeholder feedback events. The new model also incorporates the Core Fidelity Standards of Crisis Resolution services developed by University College London and the Royal College of Psychiatrists.

LYPFT are in the process of agreeing in partnership a revised and increased starting salary for newly appointed consultants which aims to be agreed by September. This post will be in scope for the new consultant salary range once agreed.

This is a 10 PA post plus a 1% category B availability for out of hours on-call duties.

Job responsibilities

Leeds Partnerships NHS Foundation Trust is seeking to appoint a full time (10 PAs) Consultant in General Adult Psychiatry (Crisis Resolution and Intensive Support Service). The work of the ISS is considered within LYPFT to be integral to the smooth transfer to and from different parts of the acute pathway.

The post holder will be one of 4 consultants providing acute care to patients under the Crisis Resolution Intensive Support Service. Although a city wide service, the post holder will work primarily from the West area of the city, the emphasis being on facilitating early discharge from the acute wards, overseeing the progression of patients through the home based treatment pathway as an alternative to hospital admission) via intensive support and ensuring care is provided in the right place of the acute pathway at the right time.

The CRISS is made up of the Crisis Resolution component based at the Becklin Centre and the ISS part of the service. The s136 suite (currently 4 bedded) and 6 bedded mixed Crisis Assessment Unit (CAU) are also based at The Becklin Centre alongside the acute wards 2 male and 2 female. There is a further male ward and 10 bedded PICU at the Newsam Centre. The CAU is designed for brief admissions (up to approx. 5 days) and is closely linked to the ISS as one of the main points of referral into and transfer from. There is an expectation that the ISS team will liaise closely with the CAU to ensure timely entry and exit from theses beds.

Although working primarily from the west base, the post holder will be expected to work flexibly when needed and when appropriate, for example assisting colleagues in other areas of the city when demand has outweighed resource or if prior knowledge of a particular patient on the Crisis Assessment Unit or in the s136 suite means the post holder is the most appropriate person to see that individual. As a core number of staff work from each locality each day, including medical colleagues, the need for flexible working across the city is infrequent and often minimised by good communication and liaison between CRISS colleagues.

ON CALL AND COVER ARRANGEMENTS

The post holder will take part in one of the Consultant on call rotas. There is a Consultant on call for the West and for the East side of the city both of which, at present are approx. 1 in 14 rota. This is a third on call rota with a Core Trainee (CT 1-3) providing first on call and a Specialist Trainee (ST 4-6) providing the majority of second on call. On-call work is paid at Category B level 1%. For urgent presentations which occur out of hours, the Leeds CRISS (crisis resolution intensive support service) supports clinical work, including MHA assessments and the s136 is within the CRISS team base and staffed by that service and the on call doctors overnight and at weekends.

There is a reciprocal arrangement for cover with the 3 other CRISS consultants and other general adult community consultants based at Aire Court.
