This is an new and exciting opportunity to join the Assertive Outreach team to facilitate healthy living initiatives addressing health inequalities within our service user population as part of the Healthy Living Service for Leeds and York Partnership NHS Foundation Trust.
We are looking for 2 motivated, innovative and enthusiastic healthy living advisors to work as part of the Assertive Outreach Team, based at Asket Croft. The Assertive Outreach Team supports service users across the city of Leeds, who have highly complex, severe and persistent mental health problems. The team works in creative and innovative ways to help people who are difficult to engage, may have a dual diagnosis and a history of risk.
The healthy living advisor roles focus on assessing, planning, and delivering healthy living interventions to service users to enable people to improve their mental and physical health.
Main duties of the job
The Healthy Living Advisor role will focus on assessing, planning, and delivering healthy living interventions to service users as part of the Healthy Living Service to enable people to improve their mental and physical health.
The post will have a focus on the delivery of interventions aimed to address the physical health inequalities faced by people with severe and enduring mental health problems.
This will include physical activity, healthy eating advice, first line weight management advice (under the supervision of a dietitian), sexual health, smoking cessation, phlebotomy, and brief interventions on drug and alcohol usage following appropriate training.
This will be delivered through advice and information for service users and staff, training and development for service users and staff, health promotion and physical interventions. The Healthy Living Advisor will encourage service users to access local opportunities and services, signposting people to activities which support health and wellbeing.
The post holders will be part of the Assertive Outreach Team and will work with service users in the community that present with severe and enduring mental illness and complex needs.

Interventions are delivered face to face on a 1:1 and group basis with emphasis on the delivery of physical activity interventions, as well as healthy eating advice, first line weight management advice (under the supervision of a dietitian) and brief interventions on smoking cessation and drug and alcohol usage following appropriate training. This is delivered through advice and information for service users and staff, training and development for service users and staff, health promotion and physical interventions.
We are looking for a practitioner who is qualified to a minimum of NVQ level 3 in an area relevant to the post (e.g. nutrition). Please note that we require the post holder to have a Level 2/3 qualification in Fitness Instruction and be CIMSPA registered.
- Deliver safe, effective and evidence based healthy living sessions specific to their area of expertise within different locations including carrying out risk assessments of facilities to be used
- Act as an advisor to health professionals on matters relating to healthy living depending on their area of expertise i.e. nutrition, smoking cessation, drug or alcohol advice
- To motivate and encourage service users to participate in therapeutic or recreational activities to promote healthier lifestyles
- To independently provide personalised healthy living programmes for individuals based on their assessment and modify them according to their need according to their area of specialist knowledge i.e. smoking cessation, nutrition, drug or alcohol use
- To contribute to the planning, delivery and evaluation of exercise and activity training
- Work alongside the Multi Disciplinary Teams to provide advice, care planning and risk assessment based on physical health needs
- To ensure safe practice at all times through the recognition and appreciation of clinical governance
- Link in with healthy living advisors across the healthy living service in Leeds and York Partnership Foundation trust, to share knowledge, skills and peer support
