Job summary
The LYPFT Learning Disability service provides multi professional, specialist health services for people over the age of 18 with a learning disability across community settings, respite and the intensive support team. The service works in partnership with key stakeholders including service users and carers, adult social care, third sector and private providers to deliver health care for people with highly complex health needs that may include; behaviours that challenge services, sensory needs, communication needs, mental health and/or dual diagnosis and complex multiple impairment / physical health needs.
An exciting opportunity has arisen for a fulltime substantive band 6 learning disability nurse to join the LYPFT Learning Disability Service in the Community Learning Disability Team (CLDT). We are looking for a dynamic community nurse whose primary role will be to work within in a Multidisciplinary team (MDT) to triage referrals into the CLDT ensuring eligibility for specialist health services. This involves liaising with service users, their carers and other people including professionals and agencies to gather comprehensive information regarding the individual and the referral reason. This is then discussed in the MDT to determine the referral eligibility for Learning Disability specialist services and making recommendations where learning disability services are not appropriate.
There will also be opportunities to undertake direct assessments and offer interventions.
Main duties of the job
The successful candidates post operates from St Mary's Hospital and line managed from within the West and South CLDT. The service runs Monday to Friday 9am to 5pm.
The post holder will provide clinical and operational supervision to junior colleagues within the team, supporting the development of intervention toolkits for nurses and other nursing resources
The LD service has clear clinical governance pathways and supports professional and service development. As a band 6 there is an expectation to contribute or lead on service development, service projects and task groups. Robust supervision structures are in place with 1:1s with support from the Specialist Nurse Practitioner, peer support through weekly MDT meetings and community nurse meetings.
The candidate should have a passion for improving the health and well-being of adults with a learning disability, using their experience of leading and delivering high quality care from relevant experience. For this role you will need to be a compassionate and person-centred clinician who can carry out comprehensive, person-centred holistic assessments to a high standard. You will need to be committed to coordinating across multi- disciplinary and multi- agency teams to promote and advocate the health needs of adults with learning disabilities. We would value a candidate who has experience of working in a diverse population and promoting social inclusion.
