Job summary

This is an exciting opportunity to be part of the ongoing development of the Leeds Mental Wellbeing Service, in partnership with both statutory and third sector services. The post holder will be linked to a primary care network (PCN), whilst providing a peripatetic specialist mental health clinical role within defined GP practices to support people with a wide range of mental health issues.

The primary aim of this team is to support General Practice in managing the mental health of those patients where an existing mental health service within Leeds has not been identified to meet their current needs. This may include offering advice to the practice or engaging directly with the patient.

Main duties of the job

The team take a transdiagnostic approach to mental health care, with the aim of using a helpful conversation/biopsychosocial assessment approach to identify and formulate the patient's difficulties. We work alongside the patient to explore their current experiences in relation to their mental health, identify their current goals and priorities, and consider what support may be helpful to best achieve these.

This can include identifying existing services which may be of benefit to the patient and supporting referrals to these or using psychoeducation and/or practical coping strategies and skills to support the patient with symptom management.

They will work closely at the interface with secondary care mental health services, and provide clinical leadership to the Primary Care Network.

The post holder will provide access to triage, specialist mental health assessment, risk assessment, and brief intervention to prevent deterioration in mental health requiring a referral to secondary mental health services. The post holder will also provide access to triage, assessment and decision support for primary care colleagues to ensure that service users with complex mental health needs access the correct pathway and intervention to meet their needs.

This will build upon Leeds' vision for the development of a new mental health framework for care in Leeds wrapping specialist mental health clinical knowledge and skills into primary and community settings.

Knowledge, training and experience Recognised professional qualification RMN, RN (Mental Health), OT, Social Worker Will maintain current specific professional and legal standards of practice in accordance with professional codes of conduct. Extensive experience of working in partnership with service users, carers and other agencies. Extensive experience in undertaking specialist mental health and risk assessment, will demonstrate that these skills are maintained and shares those skills with other team members. Will be able to demonstrate a high level of understanding of the national and local mental health and social care agenda and be able contribute to service development initiatives. Will be able to demonstrate a working knowledge of clinical, teaching and leadership skills and can demonstrate evidence to support same. Will be highly skilled in all aspects of clinical communication including the maintenance of accurate records To demonstrate and apply a highly skilled level of clinical and professional supervision. Patient/Client Care Provide comprehensive mental health assessments, formulation, skilled brief interventions, and decision support to primary care colleagues to ensure that service users with complex mental health issues are supported to have their holistic needs met in a primary care setting. Be able to respond appropriately to unpredictable situations of risk and manage these in a safe, professional and holistic manner utilising risk assessment, relapse prevention interventions and safety planning. Provide access to triage, assessment and decision support for primary care colleagues To maintain a commitment to person centred care and to promote the use of the recovery model. Actively engage service users, carers/family members, and other professionals/partners in developing collaborative care plans and crisis plans to meet service user needs. Liaise with and establish effective working relationships with a range of staff and clinicians in primary and community health services, social care, independent and third sector to meet service users social, physical and mental health needs.

7. Policy and Service Development Implementation Participate in service audit, evaluation and review. Contribute to the development of the service applying NICE guidelines through Clinical Governance and professional standards.

8. Responsible for Financial and Physical Resources To make effective use of all available resources under the guidance of the Locality Manger and Senior Primary Care Mental Health Practitioner

Adhere to Trust financial instructions eg: mileage and expenses claims, timely submission of attendance sheets and activity recording.

9. Responsible for Human Resources. Will provide supervision and appraisal as appropriate.
