Job summary

We are currently looking for a full time Band 5 Memory Nurse in the East/North East community locality. Newly qualified applicants are welcomed. This locality is based near Chapeltown in Leeds. We are looking for committed and motivated practitioners who want to consolidate their nursing practice in memory services and develop their skills, experience and knowledge. Memory Assessment Nurses are responsible for assessing service users for signs of organic progressive disease (dementia). The findings of assessment are discussed with the wider MDT with regards to a potential diagnosis (including appropriate plan regarding this) and a supportive care plan for the service user and their carer/family. As part of the pathway and nursing duties, a Band 5 MAS nurse would offer post diagnostic support, which includes practical information, signposting and emotional support after diagnosis.

Main duties of the job

MAS nurses undertake their work in both clinics and as home visits. Service users referred into the service follow the pathway through the service, to which nurses are at the front end and will complete the initial assessments for potential dementia. All service users are discussed in the weekly multi-disciplinary meeting to consider if an diagnosis is indicated and what the recommended course of action is to deliver this. Following service users receiving a diagnosis, MAS nurses are then fully competent and trained in delivering post diagnostic support which is a fundamental part of the pathway. This involves delivering information to service users and their carers/families about their diagnosis, practical support, considerations for the future, signposting and emotional support.

The successful applicant will be expected to demonstrate the capacity to work independently but under the supervision of senior team members. You will receive clinical supervision / mentorship and a robust personal development plan to support and develop your practice.

It is desirable that you can demonstrate experience and knowledge with memory assessments and evidence based tools used for diagnosis. You will be able to demonstrate an in depth understanding of the needs of service users with dementia and have an ability to support service users and their families/carers practically and emotionally post diagnosis.

MAS Nurses undertake their work in clinics and home visits. There is flexibility allowed in remote working for documentation and administrative tasks. It is essential that you have a full UK driving licence and use of a car. Successful applicants will be offered a comprehensive continuing professional development package which will include access to appraisal, clinical and managerial supervision.

Service operating hours are Monday-Friday 9am-5pm

MAS nurse duties involve initial assessments for possible dementia and post diagnostic support sessions.

The post holder is expected to demonstrate effective written and verbal communication skills, with a wide range of people. This includes high quality documentation such as initial assessments, letters to GPs and relevant professionals and risk assessments/management plans.

As well as skills in assessing dementia, the post holder is expected to have skills in assessing other mental health difficulties, such as depression, anxiety, and psychosis. And be able to deliver evidence based care guided by the individual needs of the service user. There should also be an understanding of MHA, MCA and DoLs, and an expectation to work closely alongside adult social care, primary care and third sector services.
