Job summary

We are looking for a motivated, forward thinking Perinatal Mental Health Care Co-ordinator to join our expanding Leeds Perinatal Community and Outreach Service. The service provides a range of care packages for women suffering from a moderate to severe mental illness in pregnancy and up to 1 year postnatally. This includes, though is not limited to, a specialist holistic assessment of need for mother and infant, group work, tailored 1:1 therapies and advice to a range of professionals on the care and management of women suffering from a significant mental illness or at risk of developing a significant mental illness in the perinatal period. We often work very intensively with mothers at time when their need is very acute. Some of our families have very complex social circumstances and this needs sensitive handling alongside the management of the mental illness and the ongoing safeguarding of mother and infant.

The successful candidate will be a post graduate with experience in perinatal mental health. They should have experience of working autonomously and as part of a multi-disciplinary team with an ability to think strategically and demonstrate excellent time management skills. This is an exciting opportunity to join a team that is committed to working in a multi agency recovery focussed way with mums, families and wider social networks across all areas of our service.

Main duties of the job

  • To participate in multi-disciplinary and multi-agency working, being aware of colleagues' caseloads and providing support to others as necessary and contribute to the on-going development of the Leeds Perinatal Service.
  • To maintain comprehensive and up to date written records of assessments, CPA plans, evaluations and on-going process notes in line with Trust Policy and to share these with the MDT and external agencies as appropriate.
  • To ensure effective communication and collaboration with others through attendance at, and chairing of CPA meetings, team meetings, ward rounds, clinical meetings and meeting held by partner agencies aimed at addressing the welfare of the child.
  • To ensure the service user and their families are central to and fully involved in the plan of care and have copies of care plans. I
  • Imparting sensitive and complex information to service users and their families.
  • To be aware of and adhere to all professional and Trust Policies including the reporting of incidents and accidents, complaints procedures and the management of confidential information.
  • To work collaboratively and communicate effectively across the primary care secondary care and children's social care agencies.

Job responsibilities

To work as a member of a multi-disciplinary community mental health team offering a comprehensive perinatal mental health service to women experiencing mental health difficulties.

This post is a city wide Leeds post and will involve the care and support of women suffering from acute moderate to severe perinatal mental. It includes caring form mothers and their infants either, discharged from the Yorkshire and Humber inpatient service for a minimum of 3 months and on occasion, up to one year post discharge, or mothers resident in Leeds who need specialist interventions in the community within the perinatal period to prevent hospital, admission.

On occasion the post holder may be required to support staff on the Mother and Baby inpatient setting.

To participate in the development and maintenance of a quality city wide perinatal service

through effective collaboration, liaison and prioritization with statutory and non-statutory organizations. Contribution to the building up of local care networks for mothers across the city of Leeds.

On a rota, manning a duty desk that covers the Yorkshire and Humber region.

To work as a Care Co-ordinator in partnership with service users and other Professionals with-in the Care Programme Approach framework managing and co-ordinating complex packages of care for both mother and infant.

To act as an autonomous senior clinician being accountable for a defined caseload.

To manage self and practice in accordance with the Nursing and Midwifery Council Code of Professional Conduct / The British Association and College of Occupational Therapist/ Social Work England, recognizing limitations, demonstrating ability to practice within ethical and legal frameworks with clear understanding of service users, and their families' civil rights.

To support, supervise and contribute to the professional development of members of staff working in health and social care regarding perinatal mental health.

To participate in the mentorship, education and supervision of students from a range of disciplines.
