Job summary
An exciting opportunity has arisen for Band 5 Community Learning Disability Nurse to join the Leeds and York Partnership Foundation Trust Learning Disability Service. We are looking for dynamic Nurses keen to work with learning disabled adults who require specialist health support due to complex physical, sensory, communication and behavioural impairments.
The service runs Monday to Friday 9 to 5. All team members have access to full remote technology (phones and laptops) to support smart ways of working which are paramount under the current Covid situation. LD services are adaptive in safely and effectively meeting the needs of our service users and carers.
The successful candidate will be based within the larger of our two Community Learning Disability Teams (CLDT) which covers the West South area of Leeds; you will be part of an integral part of this multi-disciplinary team (MDT). You will receive operational management from a senior nurse and will be part of a group of nurses who be in receipt of clinical leadership within the community services.
The LD service has clear clinical governance pathways and supports professional and service development. Both in- house and accredited training opportunities are available in areas such as Positive Behaviour Support, Armadillo, SAPROV and Mini Pass-Add as well as leadership courses such as Edward Jenner Programme. As a trust we also offer a full preceptorship package for newly qualified staff.
Main duties of the job
If you are a Nurse interested in supporting people working closely within an MDT and liaising with multi agency services, in advocating the health needs of this vulnerable client group this could be the post for you.
- Provide direct clinical care to adults with a Learning disability with the aim to improve service user's lives and prevent admissions to specialist Mental Health and general health hospitals by ensuring that the right plans are in place.
- You will be supported to work autonomously within your scope of practice.
- Hold a caseload including Care co-ordination to ensure the individual, family and/or carers are supported to implement care and treatment as recommended.
- Be part of a wider multi-disciplinary team working with a range of health and social care professionals.
- Involvement in assessment, planning, implementation, and review in line with the CLDT Clinical Pathways.
- Facilitation of the pathways, including Complex Physical Health, Dementia, Positive Behaviour Support, Vulnerability and Offending.
- Involvement in development of intervention toolkits for nurses.
- Provide support to Student Nurses, Senior Support Workers and Therapy Assistants.

This Recruitment Bonus is available to all externalexperienced Nursing applicants (not preceptees) and will be paid £500 upon commencement with LYPFT, and then £500 after a qualifying 6 months in post. Unfortunately this bonus isnotavailable to existing LYPFT staff.
