An exciting opportunity has arisen to join our senior leadership team in Older People's Services.
We are looking for a dynamic Implementation Manager to work in close partnership with our Care Homes Service to evaluate our existing service provision with a view to delivering service improvement. You will support with collating and sharing best practice to support bid development and to deliver support for new services through implementation, transformation and beyond.
This is a fixed term position for one year
Main duties of the job
The role will include reviewing both elements of the existing Care Home service function (standard and intensive), implementation planning, evaluation of impact and ensuring effective use of resources and any workforce strategies.
A key responsibility will be to facilitate the clinical team to develop in line with Enhanced Health in Care Homes Framework. This is part of a national drive to improve links between primary and community care in the clinical offer to people living in care home covering all aspects of health, including mental health and dementia. This will involve good collaborative working with the team and partner organisations.
We are looking for someone who is confident and comfortable working across services with external stakeholders whilst bringing a knowledge and expertise in the mental health in the care homes sector.
The ability to demonstrate previous service improvements and developments necessary for this role.
