Job summary

Do you have an interest in working with people with dementia? Are you interested in working with other health care professionals from different organizations in a community setting? Would you want to be at the forefront of a new service aiming to provide "wrap around" care in a newly commissioned bed base? If so, then this may be the opportunity for you to make a positive difference.

This is a brand new exciting dynamic service that has been designed to meet the needs of older people with Dementia and we are looking to recruit individuals who have the passion, skills and experience to work with older people who have a Dementia.

Main duties of the job

We are keen to recruit into the following position

  • Registered Mental Health Nurse at band 5 or
  • Registered Occupational Therapist at band 5

The Willows is a 10 bedded unit for people who following discharge from hospital would present with behavioural and psychological symptoms of dementia and require 24 hour care, you will work in partnership with staff from Local City Council and Leeds Community Healthcare.

The role of successful candidates will be to support staff who works permanently within the unit, providing advice and a range of person-centred interventions based to meet the individualised needs of the person with dementia.

The team have defined tasks, roles and responsibilities that includes ongoing assessment and the development of person centred care plans and providing inreach to our inpatient service to contribute to MDT discussions and support the referral of people with dementia to service
