The LYPFT Care Homes Service is seeking an experienced Band 5 Registered Nurse to join a small, friendly service that directly supports older people with complex dementia /mental health needs residing in a Leeds care home. The service is provided by registered Nurses and Occupational Therapists with a background and specialist knowledge and skill in mental health & dementia. Broadly, the model of service delivery includes: care coordination; timely assessment, role modelling appropriate behaviours / techniques, education for care staff tailored to the needs of the individual service user, fast access to mental health, psychology and pharmacological advice.
The job requires successful applicants to provide tailored, clinical assessment and interventions to care home residents presenting with challenging and complex needs. Through multidisciplinary working, we provide care homes with comprehensive care plans, and support them in to implementing changes to improve mental health & wellbeing, and to reduce risk.
The intensive care homes treatment team works with a shared caseload of service users needing responsive and urgent assessment, support, and interventions. The team provides support during the transition from hospital to care home and to prevent unnecessary hospital admissions.
Main duties of the job
The job requires successful applicant to undertake the role of Care Coordinator and to provide tailored, clinical assessment and intervention to care home residents to support care homes with more complex residents.
We have recently embraced the CLEAR dementia strategy, developed in Northern Ireland and our ethos is underpinned by person-centred dementia care. You will be part of an MDT consisting of enthusiastic occupational therapists, nurses and psychologist, with medical support from sector Consultant Psychiatrists and their colleagues.
These are challenging and rewarding roles requiring liaison with care home staff, social workers, inpatient wards at The Mount (Older Peoples Mental Health Inpatient Unit) and LTHT (Leeds General Hospitals), as well as direct contact and assessment of older people in care homes and liaison with their families and carers
The Intensive Care Homes Treatment service operates over 7 days, 08.00-20.30 hours weekdays and 10.00-18.00 hours weekends and bank holidays
