Job summary
An exciting opportunity has arisen to join the newly established Virtual Wards and be part of a friendly, supportive multi-disciplinary team. This post forms part of the Virtual Ward implementation across Derbyshire and is supported by NHS England and the local ICB. The primary focus for the Virtual wards, will be early supported discharge and admission avoidance for cardiology, frailty and respiratory patients. The Virtual ward support the use of remote monitoring of patients using apps, technology platforms, wearables and medical devices such as pulse oximeters.
This post will be part of a seven-day service based at Chesterfield Royal Hospital but may include some off site/community integrated working. This advert is 4 posts across the specialities, please indicate on application which position you are applying for either:
- Frailty (2 full time positions)
- Cardiology (1 part time position)
- Respiratory (1 full time position)
This post will include joint working across all the Virtual wards mainly at weekends.
Main duties of the job
- Providing specialist clinical practice within a defined clinical area/caseload in a patient centred, clinical and effective way, in line with Trust policies, procedures and national guidelines.
- Working independently with support/guidance from multi-disciplinary team (MDT) as required.
- Provide the assessment of care needs, the development, implementation and evaluation of programmes of care and the setting of standards of care.
- Working collaboratively, actively, supporting and liaising with other health, social professionals and other agencies across primary, secondary and tertiary care as appropriate.
- Contributing to the development and enhancement of the service, ensuring cost effective use of resources.
- Providing expert advice, support and education within speciality area of practice to patients, significant others and colleagues within the team.
