The hours for this post are 32 Hours Per Week
To work as part of an Integrated Discharge Team including the Transfer of Care Hub at Woodside to facilitate proactive assessments for inpatients in order to facilitate a safe and effective Discharge from hospital and prevent avoidable readmission and to support with effective patient flow.To provide high quality care and support for complex people leaving hospital.Appropriately identify the persons discharge pathway, ensuring a Home First and patient focussed approach.Provide advice and guidance to the wards regarding discharge pathways and referral processes.To build strong working relationships with partners and other teams in the Trust to ensure safe and timely discharge from hospital.To work flexibly, under the direction of the IDT Nurse Manager and Social Care Managers to respond accordingly to trust pressures.To work across 7 days on a rotational basis -Shift Patterns - 8am - 4pm / 10am - 6pm
Main duties of the job
To provide high quality care and support for complex people leaving hospital.As a member of staff, you must be committed to the delivery of high quality services for our patients, including their right to be treated at all times with dignity and respectTo be able to use IT Equipment on a daily basis, using Word, Excel, Meditech and many more to support the Service.Be responsible for processing, receiving and screening the single referral as appropriate.To provide follow up phone calls to patients discharged home within 24 hours to ensure successful and non-problematic discharge.Communicate effectively and appropriately, demonstrating professional behaviour at all times.
