The Nurse Team Leader on A4 Ward plays a key role in delivering safe, compassionate, and personcentred care for older adults with complex and often changing health needs.

The postholder provides visible clinical leadership, supporting the coordination of daily ward activity and ensuring high standards of assessment, care planning, and evaluation. They act as a role model for best practice in the care of the elderly, promoting dignity, independence, and a holistic approach to patient wellbeing.

A central part of the role involves supervising and guiding nursing and support staff, fostering a positive and inclusive team culture. The Nurse Team Leader supports staff development through mentoring, coaching, and constructive feedback, helping to maintain a skilled and confident workforce. They contribute to effective communication across the multidisciplinary team, ensuring smooth patient flow and safe discharge planning.

The postholder maintains high standards of documentation, medication safety, and adherence to clinical governance, safeguarding, and infection prevention requirements. They work closely with patients, families, and carers to ensure involvement in care decisions and to enhance the overall patient experience on A4 Ward.

This role is vital in supporting service improvement, quality initiatives, and the ongoing development of the Care of the Elderly service.

Main duties of the job

The Nurse Team Leader on A4 Ward supports the delivery of safe, effective, and personcentred care for older adults with complex health needs.

The role involves coordinating daily clinical activity, ensuring that assessments, care plans, and evaluations are completed to a high standard and reflect each patients physical, emotional, and social needs.

The postholder provides clinical leadership, supports decisionmaking, and ensures timely escalation when patient conditions change.

A key responsibility is supervising and guiding nursing and support staff, promoting a positive and supportive team culture. This includes organising staffing, allocating duties, leading handovers, and contributing to staff development through coaching and feedback. The Nurse Team Leader encourages evidencebased practice and helps maintain a learning environment across A4 Ward.

The role requires maintaining accurate documentation, ensuring safe medication administration, upholding infection prevention, safeguarding, and clinical governance standards. Working closely with the multidisciplinary team, the postholder supports effective discharge planning, reduces avoidable harm, and promotes patient flow.

Communication with patients, families, and carers is central to the role, ensuring dignity, involvement, clear understanding of care plans. The Nurse Team Leader also contributes to audits, quality improvement, and service development to enhance the overall performance of A4 Ward.
