Job summary

Band 6 Neonatal Outreach Nurse

22.5 hours per week

We are currently looking to recruit an experienced neonatal nurse who is dynamic and innovative to work in our neonatal outreach team. This service provides nursing care and support to babies and families post discharge from the neonatal unit. You will need to have excellent organisational, communication and team working skills in addition to neonatal nursing care skills.

The post holder will be working as part of a small team of neonatal nurses delivering care in the patients home. They will also be part of the larger neonatal team and will be actively involved in discharge preparation. To ensure a smooth transition from hospital to community care you will be required to work a combination of shifts in both the neonatal unit and community. The neonatal team is a friendly and highly experienced team working with babies and families in Rotherham.

The post holder will work in liaison with the community childrens nursing team, neighbouring neonatal services and universal services. The neonatal unit is a 14 cot unit with 2 IC and 2 HD cots, providing a Local Neonatal service. The unit is part of the Yorkshire and Humber Neonatal ODN. This is an exciting opportunity to develop the neonatal service providing seamless care and facilitating earlier discharge home.

Main duties of the job

The main duties are to co-ordinate the safe discharge of patients from the neonatal unit in liaison with the nursing and medical team.As a community nurse the post holder will provide care and support post discharge for a defined period of time and will support the family to transition to continuing care by universal services or other specialist services.

The post holder will work as part of a small team working in conjunction with the MDT.
