Job summary
This role involves working
within the Community Therapy Team pathways which includes In-reach to Community
Hospital Admission Avoidance Team, Integrated Urgent Response at Home and
Integrated Rehabilitation & Reablement at Home. There will be an
expectation of flexibility to work throughout the Community Therapy Team.
Worked over 4 long days - 9hr on Sunday9hr 30 on Monday - Wednesday
Main duties of the job
The role will specifically provide the following:
- Independently provide assessment for patients under the care of the Urgent Community Response Pathway.
- Implement and evaluate therapy interventions for patients relating to their treatment in line with specific condition pathways.
- To input accurate patient data onto the IT system.
- To organise and plan own caseload and others to meet service and patient priorities. Readjusting plans as situations change/arise in consultation with the team where appropriate.
- Leadership with delegation, support and supervision of therapy staff where required e.g. induction of new staff/student mentorship.

- Diploma/Degree in Occupational Therapy
- Due to the nature of the role it is essential that applicants are able to travel independently between TRFT sites and for patient visits in the Rotherham area and have access to a suitable vehicle for business purposes. If necessary, adjustments can be considered in accordance with the Equality Act 2010

- Membership of Professional body
- Postgrad qualifications relevant to Occupational Therapy/Rehabilitation
- Completed recognised IT course e.g. ECDL/Elite/MOST
