Job summary
We are looking for an experienced Occupational Therapist to join our established Urgent Therapy Team, working within Care Group 4. This team provides therapy for adults with a wide range of conditions within their home/residential settings. As part of the wider Community Therapy Team, the Teams role is to prevent admissions, aid in facilitating timely assessment and intervention on discharge. Assist with crisis management providing assessment and intervention. The team includes Physiotherapists, Occupational Therapists and Therapy Support Workers. You will work closely with a wider MDT of nurses, nursing support workers, social workers, voluntary sector and care staff. Supporting the Virtual Ward meetings and the Transfers of Community Care Hub with clinical reasoning and decision making. The team provides a 7 day service and there will be an expectation to work weekends, predicted pattern (Sunday - Wednesday). The successful candidate will have experience working in a community setting and be used to a holistic approach to assessment and planning with a very patient centred ethos. Other requirements are good clinical risk management, equipment assessment and provision, and excellent communication skills. We have a strong commitment to supervision, PDR, CPD, student and staff training
Main duties of the job
This role involves working within the Community Therapy Team pathways which includes In-reach to Community Hospital Admission Avoidance Team, Integrated Urgent Response at home and rehabilitation at home. There will be an expectation of flexibility to work throughout the Community Therapy Team. The role will specifically provide the following:
- Independently provide generic therapy assessment for patients under the care of the Urgent Community Response Pathway.
- Implement and evaluate therapy interventions for patients relating to their treatment in line with specific condition pathways.
- Provide specific Occupational Therapy assessment with regards to equipment provision, adaptations, moving and handling risk assessments and onward referrals. Following a PEO model of practice with a patient centred approach.
- To input accurate patient data onto the EPR system.
- To organise and plan own caseload and others to meet service and patient priorities. Readjusting plans as situations change/arise in consultation with the team where appropriate.
- Leadership with delegation, support and supervision of therapy staff where required e.g. induction of new staff/student mentorship.
