A number of exciting opportunities has arisen within the Corporate Operations Team, for Community Ready Discharge Coordinators.
Main duties of the job
The successful candidates will need to offer a proactive and responsive approach to ward based patient flow and discharge. You will play an important role supporting clinicians to facilitate safe and timely discharge from for our patients. This role will be covering all wards, including cover for the Community Ready Unit and across the hospital, specifically those within the care groups of surgery and integrated medicine. Duties include but are not exclusive to: Liaising with patients, their family members Working closely with the multidisciplinary team Making referrals to appropriate services Working with IT systems and EPR daily Tracking Patients journey and recording any potential delay We will offer a supportive working environment in which to continue to develop your skills, and an in-service buddy system will be used. The NHS provides enhanced rates of pay for weekends and bank holidays, which is even paid within your 3-6 week training period. The successful candidate will need to be diplomatic, self-motivated and enthusiastic with a professional attitude, the candidates will be from a Healthcare or Hospital/ Community Services Administration background. You will be able to demonstrate good interpersonal and communication skills, together with the ability to work as part of a multidisciplinary team, effective time management skills, and the ability to organize self and others.
