Job summary
37.5 hours per week
We are looking to appoint an Advanced Clinical Practitioner (Band 8A) within our Community CSU, this role will be working within our community respiratory service and virtual ward.
Our ideal candidate will have a strong background and passion for respiratory care and have experience in managing acute and chronic illness episodes for patients within a clinic and community setting. We will also consider candidates within other backgrounds who are looking to develop within this field.
Within our Community CSU within Care Group 4, have established multi-disciplinary teams, made up of community physicians, Nurse Consultants, ACPs and trainee ACPs, specialist nurses, healthcare support workers and therapy teams. We are passionate about providing care to patients in community and home settings. Working with partners to support admission avoidance early discharge from hospital. This role will be integral to this way of working, reflecting the national agenda bringing care closer to home.
We are a care group within the wider NHS Foundation trust allowing for the development of efficient pathways for our patients, collaboration of services and opportunities for our colleagues that comes with being in a foundation trust.
This role will work across our community sites with both the Community Respiratory Team and our Virtual Ward team.
Main duties of the job
To work supporting patients with acute and chronic respiratory conditions. Supporting teams with clinical knowledge and guidance on management plans for these patients. Working with patients and partners to support care in the patients home and community settings.
Our successful candidate will be involved in training and supporting junior colleagues and undertaking clinical supervision with a clinical team.
