Job summary
We are looking for an experienced Physiotherapist to join our established Community Hospital Admission Avoidance Team (CHAT) based within Urgent and Emergency Care Centre (UECC) within Rotherham NHS Foundation Trust. This team provides therapy for adults with a very wide range of conditions in the hospital setting. As a dedicated part of the wider Community Therapy Team, the CHAT team in-reaches into the UECC, to prevent admissions. The team includes Physiotherapists, Occupational Therapists, Assistant Practitioners and Therapy Support Workers and works closely with a wider MDT of nurses, nursing support workers, social workers, voluntary sector and care staff. The team provides a 7 day service and there will be an expectation to work weekends, predicted pattern (Tuesday - Saturday). The successful candidate will have experience working in a community setting and be used to a holistic approach to assessment and planning with a very patient centred ethos. Other requirements are good clinical risk management, equipment assessment and provision, and excellent communication skills. We have a strong commitment to supervision, PDR, CPD, student and staff training. The successful applicant will have a full driving license with access to a vehicle for business use and a valid work permit to be eligible to work within the UK. The post is subject to Occupational Health Screening and Criminal Records Bureau Disclosure checking.
Main duties of the job
This role involves working within the Community Therapy Team pathways which includes In-reach to Community Hospital Admission Avoidance Team, Integrated Urgent Response at Home and Integrated Rehabilitation & Reablement at Home. There will be an expectation of flexibility to work throughout the Community Therapy Team. The role will specifically provide the following: Independently provide assessment for patients under the care of the Urgent Community Response Pathway. Implement and evaluate therapy interventions for patients relating to their treatment in line with specific condition pathways. To input accurate patient data onto the IT system. To organise and plan own caseload and others to meet service and patient priorities. Readjusting plans as situations change/arise in consultation with the team where appropriate. Leadership with delegation, support and supervision of therapy staff where required e.g. induction of new staff/student mentorship.
