An exciting opportunity has arisen for an Allied Health Professionals or a Registered Nurse to develop the transitional care/developmental appropriate health care at The Rotherham NHS Foundation Trust. This is a secondment/fixed term opportunity for a year to spring board the transition journey at the Trust, supporting both colleagues at the organisation alongside young people and their families in relation to what does effective and supportive transitional care look like for the population to Rotherham. Working Trust wide to support across all the Care Groups and wider local stakeholders in defining the support and care that young people and their families need to support their health care. Coproduction will be a key element of this role.

Main duties of the job

As new roles these will be key in determining the training and educational nerds to the Trust to understand what is needed to support young people and their families to support their health care and creating a training programme to support this. Linking closely with the work that is being undertake both at a Place level and across the South Yorkshire footprint. Working closely with young people and their families to understand what support they require to ensure that they meet their health care needs and defining a blue print for service delivery which supports the Trust's Transitional Strategy. Being able to create both qualitative and quantitative data to demonstrate what the impact that transitional health care can have on the population of Rotherham will be fundamental.
