Job summary

Working as part of a multi-disciplinary health and care team the role holder will be part of our multi-disciplinary urgent community care team. You will provide high quality senior nursing support for people requiring an urgent community response, enabling them to be cared for in their own home thereby reducing avoidable admissions and supporting timely discharge from the acute and community bed setting. You will provide clinical leadership to the members of the Urgent Community Response Team. You will have responsibility for overall management of patients and caseloads. You will assess and make decisions regarding patient care and safety in a crisis situation as part of the Transfer of Care Hub. Support the Service Leads with supporting and leading the staff. This will include completing personal development reviews, sickness and absence monitoring and completing clinical audits as appropriate to the service.

Main duties of the job

Will utilise their clinical knowledge within the Transfer of Care Hub The Post Holder will assess the individuals care needs and coordinate any required support between other services. To be competent in the use of system one, meditech and sepia and to utilise these systems as an integral part of the daily workload. Act as an advocate for the patient and family, liaising with other professionals in primary and acute care as necessary. To promote good team working and an environment to enhance job satisfaction and ensure team members feel valued and empowered. Complete effective clinical triage for admissions avoidance and urgent response services including the virtual ward, 2 hour urgent response and step up and step down discharge to assess pathways Be involved in complex decision making and problem solving relating to clinical care and risk assessment on an ongoing basis e.g. patients with multiple and unusual care needs, complex discharge plans, resolution of complaints, first visit assessments
