Job summary
This is an exciting and challenging opportunity for an established Advanced Practitioner to work with the Urgent Community Response Service supporting the 2 hour/Integrated Rapid Response Services to the community of Rotherham. The post holder will be responsible for autonomous clinical assessment, diagnosis and treatment of patients in the community.
You will be required to utilise advanced clinical skills to assess, diagnose and treat patients requiring a clinical service. The post holder will act as a role model and provide clinical leadership to the Multi-Disciplinary team including The Urgent Community Response Team members. Our aim is to deliver high quality care to help reduce the demand for hospital attendance and admission , managing patients in their own home, working closely with GPs and other health care professionals
Advanced Practitioners at The Rotherham NHS Foundation Trust are supported to develop against the 4 pillars of advanced practice by way of access to the Practitioners E-Portfolio (PEP) and dedicated non-clinical time.
Main duties of the job
Be a highly autonomous Advanced Clinical Practitioner working independently with support when required from a medical practitioner
Aim to prevent unnecessary attendance at A&E and reduce avoidable hospital admission by delivering care in the community setting wherever possible.
Organise the clinical management of individual patients, using expert clinical knowledge and advanced skills to assess, diagnose, plan, implement and initiate changes in management where required. This will involve the monitoring and evaluating the response to treatment through analysis and interpretation of clinical tests and investigations.
Conduct Comprehensive Geriatric Assessments when required
Conduct advanced health assessments and clinical examination of patients with highly complex presentations and make clinical decisions and independent clinical diagnosis based on these assessments.
Communicate very complex, sensitive or contentious information to ensure that patients are fully informed and consent to treatment plans, this might include having sensitive discussions regarding Do Not Attempt Cardiopulmonary Resuscitation, Advanced Care Planning, life limiting diagnosis and end of life discussions.
