Job summary
To contribute to excellent patient care within the community environment by undertaking delegated clinical tasks. To work as part of the Urgent Community Response and Virtual ward Team by undertaking a range of nursing interventions following the completion of the appropriate training and assessment of the post holders competence.
Main duties of the job
To deliver high quality care to patients in a community setting ensuring that effective communication is utilised at each intervention. To work as part of a multi-disciplinary team to provide appropriate care interventions to patient's in a community setting. To work independently to deliver care interventions to promote a patient's independence as identified by the qualified nurse or community therapist to ensure that a patient's maximum rehabilitation outcomes are achieved To utilise electronic health care records to ensure that written documentation reflects each episode of the patient's care. To deliver clinical interventions to patient's in a community setting. These will include female catheterisation, catheter care, administration of insulin, oral care, and personal care. This is not an exhaustive list. Deliver care to patients and their families in a crisis situation to enable them to remain in a community setting. Support and care for patient's that are suffering from a palliative condition or are nearing the End of their life by demonstrating effective communication and listening skills. The post holder must ensure that they escalate any concerns to the qualified senior nurse.
