An exciting opportunity has arisen for a passionate and enthusiastic Band 4 Nursing Associate to join our established Learning Disability and Autism Team within The Rotherham NHS Foundation Trust.
The team offers care and support to children and adults who have a Learning Disability and or are Autistic. The team offers support to children and adults using any of the Trusts facilities and within the community to work to minimise unnecessary admissions to hospital and making reasonable adjustments to find coming to hospital challenging. As a team we support all aspects of physical health, social care needs, and risks are jointly managed by the multi-disciplinary team, forming part of an integrated holistic system approach to care.
You need to have a genuine passion in working with people with a learning disability and autistic people and willingness to make a difference in their lives. This role is pivotal in supporting people with a learning disability and autism to access physical health care. You will be expected to deliver high standards of care. This will involve giving both physical/direct support to individuals in various settings of the acute Trust, for example as an inpatient, outpatient appointment, Urgent and emergency care attendance or planned surgery/investigations. The role will also involve visiting people in their own home, or supported environment.
Main duties of the job
As a Nurse Associate you will be involved with the direct planning of care for both children and adults who have a learning disability and or Autism.
Supporting with communication needs, which may include making easy read material or communication aids.
Making reasonable adjustments to appointment pathways within the structure of the Trust, enabling a more positive experience for this patient group.
Provide support and guidance within an MDT around the use of the Mental Capacity act (2005) 1. Supporting people to improve and maintain their mental, physical, behavioural health and wellbeing. 2. Active involvement in the prevention of and protection against disease and ill health. 3. Engage in public health, community development, and in the reduction of health inequalities. 4. Where appropriate contributing to psychological interventions under the guidance of a registered professional. 5. Advocating for patients/service users needs and rights within Trust policy. 6. Encouraging and empowering individuals to have an optimum level of responsibility for their individual programme of care.
