Job summary
22.5 hours per week
To ensure that patients only stay in hospital for the time required to complete their acute treatment. Specifically this post will work with colleagues and ward teams to ensure that they provide and support the functions of the MDT ward.
You will work closely with all members of the multi-disciplinary team including medical and nursing staff especially the Specialist Stroke Nurse Practioners where you will be a huge asset within repatriating Stroke patients back from the Hyper acute stage of their Stroke.
Supporting the patient at ward level to ensure effective discharge planning processes are followed in a timely manner for the safe discharge of patients from hospital.
Main duties of the job
- Collaborative working and the ability to create and maintain effective working relationships with a range of health and social care colleagues in the hospital are an essential element of the role.
- You must be a good team player and able to communicate effectively with all grades of staff, work flexible hours across day shifts and be well organised providing efficient, safe discharge for our patients.
- Being at home or in a community setting is the best place to continue with a persons recovery once an illness requiring hospital care is over and reduces unnecessary time in hospital where a community or other setting is most appropriate.
- IT skills are essential as the post also involves accurately updating the Trusts electronic information systems in house training shall be given for these skills.
