The Community Ready Unit is a unit with beds and chairs for patients, a courtyard area, and designated pick up point for patients to be collected from the unit. We are a small, friendly team, comprising of a Team Manager and Deputy Team Manager, Staff Nurses, Support Workers, a Doctor, Ward Clerk & Housekeeper.
Patients are referred to the unit when they are ready to be discharged, but require items such as discharge medication (TTOs), paperwork, are awaiting the start time of their care package, or for their own or hospital transport to arrive.
As a RN on CRU, you will be responsible for supporting all members of the team to facilitate a safe discharge. During their time on the unit, it is essential that the care patients received on the wards continues, so you will be required to deliver person-centred care to each individual, distributing and checking prescribed medication and TTOs while complying with our clinical governance framework, providing personal care while maintaining dignity, and appropriate diet and fluids, until they are ready to be discharged safely.
This is achieved by liaising with many different specialities within the trust, including Discharge Coordinators, Hospital Pharmacy, Integrated Discharge Team and Hospital Transport. The role will also involve coordinating with district nurses, Care Homes and Home Care packages, as well as Patients relatives, to ensure each patient is safe and informed once they leave the hospital.
Main duties of the job
- Documenting patient handovers.
- Providing personal care for patients.
- Providing patients with prescribed medication.
- Checking TTOs and Discharge Letters.
- Confirming patients home care and care homes who are expecting them.
- Liaising with District Nurses to ensure care is booked.
- Booking transport for patients.
- Discussing discharge expectations and arrangements with both the patient and families, while answering any questions.
- Assessing patients needs, to ensure they leave the Hospital with the correct level of care.
