Job summary
The post holder will provide high quality senior nursing support for people requiring an urgent community response, enabling them to be cared for in their own home thereby reducing avoidable admissions and supporting timely discharge from the acute and community bed setting. Provide effective triage and assessment on a rota basis ensuring patients receive the right level of care, at the right time and in the right place according to the needs of the individual and within available health and social care capacity Provide skilled nursing advice, support and care to patients, their family/supporters and carers Have continuing responsibility for overall management of a patients and caseloads. Provide strong clinical leadership to the urgent nursing team and wider MDT acting as a role model at all times.
Main duties of the job
Provide skilled nursing advice and support to patients, their families and carers. Develop and maintain specialist skills such as, but not limited to, IV therapy, urinary catheterisation and syringe drivers Complete effective clinical triage for admissions avoidance and urgent response services including the virtual ward, 2 hour urgent response and step up and step down discharge to assess pathways Utilise clinical knowledge within the community urgent response hub to assess and make decisions regarding patient care and safety in a crisis situation Have a working knowledge of all discharge pathways and community services available. Be involved in complex decision making and problem solving relating to clinical care and risk assessment on an ongoing basis e.g. patients with multiple and unusual care needs, complex discharge plans, resolution of complaints, first visit assessments Be able to work and manage stressful situations that occur from both patient and staff dissatisfaction and be able to identify resolution for these situations. Ensure escalation of any issues when necessary. Promote good team working and an environment to enhance job satisfaction and ensure team members feel valued and empowered.
