Job responsibilities

Job Purpose:

The Urgent and Emergency Care Centre operates a Primary Care led front door streaming service, where senior primary care clinicians work in tandem with experienced triage nurses to complete the initial assessment of cases at first presentation, make decisions on appropriate management, referral and discharge. This includes managing cases by redirection to more appropriate services at the initial contact point; streaming into a Primary Care Stream, a hospital Same Day Emergency Care facility, or the Emergency Department triage.

As valued team members, Primary Care clinicians also provide a highly specialist resource to other healthcare staff, patients and carers and are actively involved in Clinical Governance/Effectiveness activities.

Key Result Areas and Responsibilities:

Managerial

To manage the service caseload in partnership with other practitioners to ensure quality outcomes for patients and carers.

To function as an accountable General Practitioner, managing clinical care and facilitating general management through partnership with the service, management team and other specialities.

To produce and comply with a job plan developed in partnership with the GP Clinical Lead (Primary Care & GP Out of Hours) to ensure all practice and other work is delivered appropriately and safely.

Primary Care clinicians will be expected to respond positively to changing workloads and demands, working to ensure appropriate timescales for the management of individual cases.

To manage clinical caseload diligently and timely, through efficient use of clinical time.

Primary Care clinicians will be expected to maintain an overview of the clinical demand and, if acting as a designated coordinator, make decisions in partnership with the clinician in charge of UECC, colleagues in the GP Out of Hours service, and clinical site manager to ensure the primary care provision and team to respond to changes in demand.

The post holder is responsible for ensuring that any personal equipment used within the UECC is calibrated and cleaned at appropriate intervals to ensure patient safety.

The post holder is required to take charge of the shift, sharing it with other clinicians, to ensure that the Primary Care and GP out-of-hours services meet patient needs.

To attend departmental meetings as required by the role.

Information Management

To provide medical leadership as appropriate to service requirements.

To identify and utilise appropriate best practice through benchmarking and clinical guidelines.

To develop positive, collaborative working relationships across all care team areas within and associated with the UECC.

To take ownership and provide leadership to the team in specified clinical areas, including training and mentoring new, locum and junior staff members, including GP speciality trainees and trainee advanced clinical practitioners.

To undertake appropriate clinical risk assessments and implement management strategies for risk identified.

To collaborate with the GP Clinical Lead for Primary Care and GP out of Hours and the UECC team to ensure that effective clinical governance is incorporated into the day-to-day delivery of care.

Patient/Client Care

Clinical

To provide appropriate care and services to patients in all ranges and level of health care needs.

Respond to medical/health problems presented by patients, including taking histories, diagnosing, investigating, treating and referring as appropriate.

To prescribe generically for patients as appropriate to their needs, and if non-generics are utilised, to provide appropriate clinical rationale with support from the medicines management team. To prescribe in a cost-effective manner.

To provide appropriate health education and advice to patients and carers to enable self-management and choice.

Non-Clinical

To maintain clinical records as required, either written or computer-based, in line with professional code and requirements utilising appropriate coding within the system.

To work with colleagues to manage the clinical caseload to satisfy current service requirements in line with national and local NHS targets.

To develop and maintain high levels of communication with patients, carers and colleagues ensuring effective care delivery.

To take part in effective communication processes between all team members to enable good working practices.

To ensure communication processes with acute Trust services, other colleagues and partners in care to ensure patient centred care is delivered.

Research and Development

To be actively involved in developmental processes within the service, in partnership with other health and social care professionals as appropriate.

To be actively involved and engaged in practice and wider team meetings and forums to promote communication and development.

To undertake and complete an annual appraisal.

To undertake appropriate training to meet personal and mandatory educational needs and service development needs, utilising CPD time.

To undertake service/personal development in line with CPD objectives and record activity.

To take part in appropriate audit processes within the service as agreed in line with UECC priorities and policy.

Policy and Service Development

To work in line with appropriate UECC policies and procedures including Human Resources, safeguarding children and adults.

To be conversant with all primary care, UECC and relevant Trust and CCG policies and procedures and ensure they are correctly implemented.

To deliver service development requirements in the UECC as agreed with colleagues, managers and TRFT Strategic Plan.

To share and disseminate acquired knowledge with all the UECC team through development / departmental meetings including clinical governance and incident reviews.

To develop and implement, as appropriate, guidelines, policies and procedures as required.

To actively engage in effective communication with UECC managerial support processes to ensure equity of service delivery and target achievement.

Working Conditions

The Urgent and Emergency Care Centre is a purpose-built facility incorporating all aspects of Urgent and Emergency Care. The unit is busy in line with most Urgent Care facilities and incorporates primary care services as part of the 24-hour commitment to Urgent Care.

The primary care element of the service operates a primary care led streaming service. A member of the team will be designated coordinator for each shift and will be expected to maintain an overview of the primary care stream, liaising with colleagues o maximise efficiency and proactively manage demand.

Experience

Essential

• Experience of working as General Practitioner
• Willingness to provide mentoring and support to all other clinicians where appropriate
• Evidence of continuing professional development.
• Case management experience

Desirable

• GP Trainer
• Other recognised roles in clinical education
• Awareness of Advanced Clinical Practitioner Training pathways

Ability and Skills

Essential

• Knowledge of methods of developing clinical quality assurance and evidence based practice.
• Experienced in remote management of patients
• Experience in managing undifferentiated presentations
• Evidence of up to date knowledge and skills.
• Evidence of ability to maintain and monitor high standards of care.
• Evidence of professional development and knowledge.

• Clinical consultation skills in all domains including initial triage assessment, face to face consultation, telephone consultation, video consultation, consultation of site in patients usual residence.

Qualifications

Essential

• Full Registration with the GMC with a licence to practice
• Please confirm your GMC registration number
• MBChB
• MRCGP

• Other relevant higher qualification

Education and Teaching

Essential

• Provide learning and development to other professionals in relation to clinical cases for example.
