Job summary

Rotherham Community Respiratory Service is a specialist respiratory service, which sits within Care group 4. The aim of our service is to provide the best possible, evidence based respiratory care, for the people of Rotherham. Our long-term goal being to improve health outcomes for this group of patients. An exciting opportunity has arisen for a Respiratory Nurse to join our Community Respiratory Team based at Breathing Space. You will be able to demonstrate sound knowledge and understanding of the management of COPD and other respiratory conditions. You will have had a number of years working in this speciality and ideally have experience of managing patients experiencing an exacerbation of their COPD, both in the hospital and in the community setting, however consideration will however be given to individuals with the appropriate experience in either setting. Ideally you will possess a diploma in COPD or be prepared to undertake this. In return, you will receive ongoing support and supervision from the Breathing Space senior team, who will help you to develop your respiratory skills and knowledge with an individualised development plan.

Main duties of the job

This role will involve the reviewing of patients admitted to the community respiratory service with an exacerbation of COPD or other respiratory condition, from both primary and secondary care. You will use your respiratory skills and knowledge to assess their suitability for admission onto the exacerbation pathway, with follow up by yourself or another respiratory team member in their own homes for a period up to 14 days. You will undertake specialist respiratory assessment of patients nursing needs, plan, implement and evaluate care delivery using clinical knowledge and clinical skills, ensuring their clinical condition is stable, maintaining a safe alternative to in -patient hospital care. You will work alongside senior colleagues to offer post discharge follow up clinics for patients who were admitted to hospital for an exacerbation of their respiratory condition but who were unsuitable for the exacerbation pathway. Your role will also involve the assessment and monitoring of long-term oxygen therapy
