Job summary

The Community Staff Nurse will work as part of the Urgent Community Response team within the Transfer of Care hub. The Community Staff Nurse will need to be able to prioritise urgent patient care in the community setting dependent on the patients needs and clinical condition. The post holder will be required to deliver clinical assessments and care interventions to ensure that the individualised needs of the patients are maintained. The Community Staff Nurse will work in partnership with members of the Health and Social Care Teams in the delivery of services to achieve collaborative and integrated working. The Community Staff Nurse will act as a positive role model for members of staff in the unscheduled care and locality setting. The post holder will reflect the Trusts core values ambitious, caring, and together.

Main duties of the job

  • The Community Staff Nurse must be able to triage patient calls utilising the appropriate triage template on system one, communicating with the patient to ensure that the appropriate care intervention is provided to patients in a timely manner.
  • You will need to be able to prioritise patient care dependent on the patients needs and clinical condition.
  • The Community Staff Nurse will provide individualised patient care plans ensuring high standards of clinical practice are maintained through continuing personal/professional development.
  • You will ensure high standards of clinical practice are maintained through continual professional development, research audit and clinical supervision.
  • To support and deliver care to patients in the community setting who are on a UCR caseload or have a urgent community response condition or illness.
  • The Community Staff Nurse role is to improve clinical outcomes for patients by providing clinical interventions within the community setting. This will reduce prevent inappropriate hospital admission.
  • The post holder will be required to deliver sensitive information, advice and support regarding illness, treatments and care plans of care to patients and their carers.
