You will need to have excellent communication and organisational skills, be willing to facilitate and support change in service delivery, have the ability to work autonomously and as part of an integrated skill mix team.

Successful candidates will work across the 0-19 service ensuring the delivery of safe, effective and high-quality services to a caseload of babies, children, young people and their families - using research-based evidence to inform practice with agreed professional standards.

These posts require travel between homes, schools, and community hubs within the Rotherham area.

Main duties of the job

  • To provide a consistent, effective, accessible and appropriate 0-19 Service for Children, young people and their families.
  • To participate in the needs assessment of local populations and work collaboratively to improve public health.
  • To maintain effective communication with children, young people, their families and carers and professionals and all other agencies involved in client care ensuring individualised client centred approach.
  • To have an understanding and awareness of the Nurses responsibility in safeguarding vulnerable clients, including children, young people and adults.
  • To work in collaboration with parent/carers and other professionals including statutory and voluntary agencies.
  • To be responsible for the delivery of public health and universal and targeted interventions across the 0-19 years age range. This will be delegated by the Specialist Public Health Practitioner for a defined population and will be delivered in line with the National and Local guidelines and public health priorities.
  • To seek out and offer and teach health promotion and preventative opportunities to children, young people their families and carers with the school Nurse team and promote positive health management.
  • To work with children, young people and families within the home environment, school or other identified community setting and will need to be able to travel to these locations independently.
