Job summary

The Supportive Care Support Worker will provide personalised information care and support for Patients who access the Supportive Care Team. They will enable people to navigate the health and social care system and to make choices that are best for their health and life. They will provide information and support within their competencies and refer on to appropriate services where the needs of the Patient are more complex. They will support the Lead Nurse-Supportive Care where required in supporting and guiding volunteers, co-ordinating the Butterfly Volunteer rota, delegating Patients to the volunteers and support the provision of a high quality care.

Main duties of the job

Meet with and be a single point of contact for users of the service e.g. Patients/clients, Carers, relatives and professionals, to support access to high quality information, care and support in relation to the four elements of the Supportive Care Team. This will include assessment of service users information, care and support needs, provision of verbal and written information, supportive listening, personalised care plan development and signposting to available services and escalation to appropriate team members and the wider MDT.

Be required to help co-ordinate and support volunteers within the service being a named contact for the Butterfly Volunteers. Work with the Trusts Volunteer co-ordinator following protocols and standards for recruitment, training and escalation of issues/concerns raised by the Butterfly Volunteers.

Manage own workload with the varying demands of the service

Are expected to contribute to the delivery and development of a Trust wide Supportive Care Service providing cover for the Clinical Nurse Specialist (with appropriate supervision and support) during periods of annual leave and absence, Help to support the day to day running, service improvement and development of the Supportive Care Service

For all elements of this role the post holder will work within the level 1 NICE psychology intervention.

Will support the CNSs in the inputting of data, relevant audits, education of others and Patient experience surveys.
