Qualified ACP Community Physician / Virtual Ward
Band 8a
Monday to Friday 8am to 4pm
Join Our Dynamic Team! As an Advanced Clinical Practitioner, you will work as part of an enthusiastic, proactive team providing community healthcare. Our team blends the expertise of existing community nursing teams with community medicine, our Virtual Ward and other acute and community services. We are at the forefront of providing person-centred, holistic community care, preventing unnecessary hospital admissions and ensuring swift, efficient discharges.

Main duties of the job
Supporting the care of Patients on out Intermediate Care Unit, working in collaboration with the community physician team, reviewing Patients at home and in care homes, initiating investigation and management plans as appropriate for the Patient and working within and alongside our virtual ward team. This applicant will work Monday to Friday 8am to 4pm (schedule negotiable).
Your Skills and Experience:
1. Extensive experience in healthcare, particularly in acute care, community and primary care, frailty, advance care planning, and comprehensive assessment.
2. Autonomous decision-making, communication, and advanced clinical skills.
3. Commitment to continuous learning and managing diverse workloads and environments.
