Band 4 Assistant Practitioner (Stroke Therapy)

37.5 hr per week

We have a fantastic opportunity for an Assistant Practitioner within our Stroke Therapy Team. We are a compassionate team and provide high-quality person-centered rehabilitation to stroke survivors in Rotherham. We deliver this care in a consistent and organised way to maximize the potential of the service-users we support. We are efficient and accountable, and communicate effectively with each other and our clients. We always put the stroke survivor, their carers and the people we work with, at the centre of every decision we make.

We provide a pathway of integrated care across the Stroke Recovery Unit (acute stroke and rehab) and the Integrated Community Stroke Service (early supported discharge and community stroke rehab up to 6 months).

We are the only therapy team across South Yorkshire and Bassetlaw to work in this innovative and dynamic way. In doing so, our pathway working provides a seamless transfer of care for stroke survivors and their carers. Hyper-acute stroke care is delivered locally in Sheffield and Doncaster.

This provides the opportunity to work closely with our partner organisations across our integrated care system.

Our multidisciplinary team includes Speech and Language Therapists, Occupational Therapists, Physiotherapists, Clinical Psychologists, Assistant Practitioners, Therapy Support and admin staff. This skill mix enables us to offer holistic and bespoke rehab.

Main duties of the job

We will support the post-holder to work autonomously under direction to manage and prioritise a caseload of patients.You will contribute to MDT working across the pathway in the hospital and across the community setting including groups, 24 hour reviews, and mood and cognition screens.You will receive clinical supervision and participate in regular in-service training sessions and be supported to further develop both leadership and MDT Stroke-specialist clinical skills.You will contribute to the national targets associated with this specialty i.e. SSNAP and be responsible for the coordination, delivery and running of the 6 month reviews clinics. You will also be involved in service improvement working closely with the Team Leaders and Clinical Lead in line with the National Stroke Service Model and the new National Clinical Guideline for Stroke.You will work with the team on a midweek basis and contribute to weekend and bank holiday working alongside the multidisciplinary team on a rota basis.Applicants will have:- A relevant foundation degree, or NVQ in Diagnostics and Therapeutic Care (or equivalent) with additional relevant experience in an NHS setting- English and Maths GSCE Level 4 or above (or equivalent)- A willingness to undertake training to meet the competency framework- The ability to independently travel around Rotherham and have access to a suitable vehicle for business purposes (Adjustments can be considered in accordance with the equality act 2010)
