The post holder will work as part of the Childrens Community Nursing team supporting the Specialist Educational Nurses to deliver and maintain high quality care for children who have complex needs and or learning disabilities who are attending educational establishments. They will provide support to Carers and Teachers and support Consultant community Paediatricians with school health assessments, and special school medicals which take place at school and within the community.
Main duties of the job
- Liaising with Specialist Educational Nurses, Continuing Health Care Coordinator/Community Childrens Nurses/Lead Nurse and relevant professionals in a timely manner.
- Work as part of a designated clinical care team delivering care that focuses on the direct needs of the individual.
- To work in partnership with the multi-disciplinary team to ensure a holistic approach according to the care plan under the guidance and delegation of appropriate professionals.
- To work effectively and flexibly under the direct or indirect supervision to meet the child and family individual needs
- To monitor height, weight and record this accurately onto systm-one.
- To act as a chaperone for children and young people attending special school medicals.
- Ensure clear, concise, accurate and legible records and all communication is maintained in relation to care delivered adhering to local and national guidance.
- Communicate effectively across a wide range of channels and with a wide range of individuals, the public, health and social care professionals, maintaining the focus of communication on delivering and improving health and care services.
