The Care Homes Team are an award winning, specialist multidisciplinary team who work to improve the lives of people living in care homes by supporting care homes to provide good quality health care and foster a culture of person-centred care.The service aims to prevent avoidable admissions to hospital, facilitate early discharge and prevent readmissions by providing holistic assessment and intervention, particularly around falls prevention and management, swallowing and communication assessment and management and long term conditions management. We also provide bespoke training to care homes staff on request or where needs are identified.We are looking to recruit a Speech & Language Therapist with highly specialist skills and knowledge to join our team for 1 day a week.Your role will be to provide highly specialist assessment, treatment and management of a complex caseload of patients with swallowing and communication difficulties. You will support the Lead Nurse and Therapy Team Leaders in service delivery and development, leadership and management responsibilities, as well as providing clinical supervision for the Band 6 Speech & Language Therapist.
Main duties of the job
Providing highly specialist assessment of complex elderly patients. Treatment and management of a complex caseload of patients with swallowing and communication difficulties. Supporting care homes with their management of complex residents. Providing training opportunities to both NHS staff and care home staff.You will support the Lead Nurse and Therapy Team Leaders in service delivery and development, leadership and management responsibilities, as well as providing clinical supervision for the Band 6 Speech & Language Therapist.We are looking for a passionate and enthusiastic Speech & Language Therapist who is: Keen to work in care homes. Dynamic and flexible with a broad range of experience. Experienced in working with older people with a broad range of conditions including Dementia. Able to work autonomously and have excellent communication skills.

- Recognised Speech & Language Therapy Diploma/Degree qualification or equivalent
- Due to the nature of the role it is essential that applicants are able to travel independently across the county and have access to a suitable vehicle for business purposes. If necessary, adjustments can be considered in accordance with the Equality Act 2010
- Post-graduate qualifications to Masters level or equivalent with respect to dysphagia and/or communication problems
