Job summary

A rare and exciting opportunity has become available to work within the Rotherham Alcohol Liaison Team based at The Rotherham NHS Foundation Trust. The Alcohol Care Nurse will be responsible for the care and treatment of patients who require a comprehensive Alcohol assessment. Delivering a whole needs approach to facilitate appropriate levels of care for patients including complex needs. Implementing measures which support reduction in presentations, acute admissions, reduction in length of stay, and promote recovery.

The post holder will predominantly work across the acute trust, assessing patients who are referred for brief interventions, complex case management including Alcohol Withdrawal Syndrome, seizures and delirium tremens.

This service has become highly regarded both locally and nationally due to the extensive care and support delivered to enabling patients who are assessed have safer care and support 7 days a week.

Due to the nature of the role it is essential that applicants are able to travel independently across the county and have access to a suitable vehicle for business purposes. If necessary, adjustments can be considered in accordance with the Equality Act 2010.

Main duties of the job

  1. The post holder will assess patients referred from A&E and all in-patient wards/departments as appropriate and consenting. Close attention to individuals already known to be frequent attenders will require an MDT approach as discussed at the High Intensity User Group (HIUG) Providing expert advice where required, referral to existing services, focusing on facilitating care in the community where safe to do so.
  2. The post holder will work closely with other services internal and external to the Trust.
  3. The post holder will deliver brief interventions to patients, encouraging referrals to community services/networks. Patients unable to access services will be integral to the ACT caseload.
  4. The post holder will deputise for the Alcohol Lead Nurse where required and appropriate.
  5. Actively contribute to the delivery of the alcohol care team by attending pathway development and meetings when required, supporting the Alcohol Lead Nurse as appropriate.
  6. To utilize systems that are in place to keep patients safe and manage risk.
  7. Work a 7-day shift system covering hours 8am 8pm.
