Job summary

A opportunity has arisen to join the Division of Medicine as an Advanced Clinical Practitioner in Same Day Emergency Care (SDEC).We are seeking to recruit an enthusiastic, innovative, and driven qualified Advanced Clinical Practitioners to the acute medical team from nursing or AHP background - applicants who are nearing the final stages of their MSc would also be considered.We are passionate and driven to provide the best care for our patients across Acute Medicine in the Division of Medicine.The development of these Acute Medicine roles are to support the expansion and development of SDEC. This is closely located to the Acute Medical Unit (AMU).There will be opportunities to be involved in acute medical care across the acuity spectrum; we will support you to become a rounded clinician to pursue the 4 pillars of Advanced Practice (Clinical Practice, Education and Learning, Leadership and Research). Main duties of the job

The post-holder will provide clinical expertise, leadership, consultancy and collaborations to ensure that patients gain effective, appropriate and timely care.

The ACP (post holder) will engage directly with the Consultants, medical staff and the multi-disciplinary team in the management of patients as a pivotal member of Team.

The post holder will undertake advanced clinical assessment, initiate investigations and interpret their results so clinical diagnosis can be determined and appropriate treatment initiated.

The post holder will use advanced knowledge and skills to provide autonomous practice to patients including independent clinical assessment, diagnosis, and treatment of patients with undiagnosed and undifferentiated conditions. In addition to this the ACP is also responsible for appropriately referring patients to relevant specialties for any necessary inpatient or outpatient investigations as well as facilitating safe patient discharge.
